Nutrition Service Self Pay

Step 1: Submit Proof of Income

Acceptable forms of Proof of Income are paycheck stubs and wage inquiry for the Department of Labor. Once proof of income has been reviewed, you should receive an email with an invoice showing the amount due and instructions to submit payment. Submit your proof of income documentation to: Latrice.Johnson@dph.ga.gov

Step 2: Submit Payment

Only submit payment for Individual Nutrition Counseling Services after submitting proof of income and receiving an email with an invoice and instructions to submit payment.
